Supporting Documents

For each application, there are documents required to support your proposal.

  1. Obtain approval signatures from the county judges and mayors of the locations you plan to serve
  2. Provide a detailed, line-item budget (except for Naloxone Hero and Sentinel applications)
  3. Fill out the application found on the Funding Opportunities page
  4. Provide the additional documentation required by each proposal category
